A Cliniko treatment note template for an exercise physiology group session

This template records one group exercise class as a short note repeated once per attending patient: attendance, per patient vitals and exertion, and any individual modification, plus a single shared program description. It suits Medicare group services for type 2 diabetes (items 81105, 81115 and 81125) as well as private group classes, and is built as a Cliniko treatment note template.

Build it in Cliniko

  1. Build the shared program fields and the per patient fields as one template under Settings > Treatment note templates, naming it Group exercise session, so a single template covers both what happened for the class and what happened for each attendee.
  2. Set the class record and shared program sections to appear once, then repeat the per patient section fields as a separate note against each attending patient's own file rather than one long note listing every name, since Cliniko treatment notes belong to a single patient record.
  3. A practical way to run this in Cliniko: write the shared program description once, copy it into each attending patient's note for that date, then complete only the per patient fields individually. Some clinics keep the class record as a short internal document outside Cliniko and copy the date and program line into each patient note.
  4. Type out the Multiple choice options for class type and attendance exactly as listed, so reporting on Medicare group services stays consistent across the year.
  5. Assign the template to a Group class appointment type if one exists in your Cliniko schedule, or apply it manually from each patient's file after the class.
  6. Test it by writing notes for two or three attendees from the same class date, and check that the shared program wording matches word for word across their files, since that consistency is what a Medicare or private health fund review will look for.

AHPRA: Accredited exercise physiologists are accredited by Exercise & Sport Science Australia rather than registered under the National Law, so the AHPRA Code of conduct does not apply directly. The same record keeping principle applies here as for any allied health note: each patient's file needs to show what happened to them individually, even when the session itself was shared.

A group class still needs an individual record

The hardest part of documenting a group class is that Cliniko treatment notes are built around one patient at a time, while the class itself is one event shared by several patients. This template splits the record into a shared program section, written once and reused word for word across the group, and a per patient section covering attendance, vitals and any individual modification. Writing the shared section once and copying it in keeps every attendee's note consistent, which matters for a funding body checking the same class was actually delivered to everyone who was billed for it.

Why Medicare group services shape this template

Group allied health services for patients with type 2 diabetes are billed under Medicare items 81105, 81115 and 81125, following an assessment of suitability under items 81100, 81110 or 81120, and the service must be delivered to a group of between 2 and 12 patients. That group size sits in the class record section as a plain count field, because a reviewer checking a claim against the note will look for it directly rather than infer it from a list of names. The sessions remaining field exists for the same reason: type 2 diabetes group services carry an annual service limit, and a practitioner tracking it inside the note avoids over servicing a patient by mistake.

Vitals and exertion belong to the individual, not the class

Heart rate, blood pressure and perceived exertion vary between patients doing the same circuit, so they sit in the per patient section rather than the shared program. The rating of perceived exertion field also gives an exercise physiologist adjusting intensity mid class something objective to record beyond how the session looked from the front of the room, and gives the next practitioner running that patient's class a starting point.
