ServiceM8 form

Free ServiceM8 commercial cleaning checklist form

This is a ServiceM8 Form for commercial cleaning contracts. Each task carries a frequency tag (daily, weekly or monthly), so one form covers a whole contract and the cleaner only ticks the rows due that visit. A required photo per room and a client signature turn the completed form into proof of service, saved as a PDF in the job diary.

Set it up in ServiceM8

  1. In ServiceM8 open Settings > Forms > Add Form and name it Commercial cleaning checklist.
  2. Add each question in order, matching the type shown (Choice means Multiple Choice, Multiple choice means Multiple Choice (Multi-answer)), and tick Mandatory where required is true.
  3. Stick to letters and numbers in question wording. ServiceM8 warns that special characters in questions can break PDF generation, so write the frequency in brackets as plain text, for example weekly not Weekly*.
  4. Download the sample template from the form editor, add your logo, and lay the fields out under the same section headings used above so the printed PDF reads room by room.
  5. Turn on the Recurring Jobs + Reminders add-on and create three Recurring Jobs against the client, one on a daily pattern, one weekly, one monthly, all using this same job type and form.
  6. Attach the form to the job type used by all three recurring jobs so it appears automatically on every visit, and set Service type for this visit as the first question so the cleaner orients to it straight away.
  7. Run one of each frequency as a test job before the contract starts, and check the PDF in the job diary reads cleanly with all four photos attached.

Why one form covers three frequencies

A cleaning contract usually mixes daily, weekly and monthly tasks in the same rooms: bins and floors every visit, grout and vents once a month. Building that as three separate ServiceM8 Forms means three form names to remember, three PDFs scattered through the job diary, and a cleaner who has to pick the right one before starting. This form keeps every task in one place, grouped by room, with the frequency written into the task label itself.

The Service type for this visit question at the top does the sorting. A cleaner rostered on a Tuesday afternoon selects Weekly service, works through the rows, and marks anything outside that frequency as Not required today rather than skipping it, so the client sees the full scope of the contract on every PDF, not just what happened that day.

Why photo proof lives per room, not per task

Commercial cleaning disputes are almost always about a specific area, not a specific task, so the photo sits at the end of each room section rather than after every checkbox. That keeps the form quick to complete: four required photos per visit instead of thirty, one per room, taken once the whole section is ticked off. If a client queries the reception area, the job diary has a timestamped photo of exactly that space from that visit.

Setting it up against Recurring Jobs, not a single job

Because ServiceM8 Forms cannot branch on an earlier answer, the practical setup is three Recurring Jobs against the one client and job type, one on each pattern, all pointing at this same form. The Recurring Jobs add-on creates each occurrence automatically from the job template a set time ahead of its date, so the office is not manually booking a cleaner in every week. The cleaner still only fills in one form; the recurrence pattern just decides which visits happen and when, and the Service type question tells them what to focus on once they arrive.

What this form will not do

  • ServiceM8 Forms cannot show or hide a question based on an earlier answer, so the daily, weekly and monthly rows all print on every visit. The cleaner marks the rows outside today's frequency as Not required today rather than leaving them blank.
  • The form does not total or score compliance across a contract. A site history of PDFs in the job diary is the record; ServiceM8 does not roll these into a single ongoing scorecard.
  • Client signature cannot be made conditionally mandatory for on site visits only, so it is left optional and covered by a note instead.
  • A completed form becomes a PDF in the job diary once submitted; it is not editable afterwards, so a cleaner who makes a mistake needs to submit a fresh form rather than correct the old one.

ServiceM8 Forms offers eight question types (Text, Text (Multi-line), Number, Date, Multiple Choice, Multiple Choice (Multi-answer), Signature and Photo), any question can be made Mandatory, and a completed form is converted to PDF and saved to the job diary. 2. The Photo question type in ServiceM8 Forms displays an image in an allowed format, JPEG or PNG, attached to that field. ServiceM8 Help Center, checked 25 Sep 2026.
  3. Recurring Jobs in ServiceM8 are for regular repeating work with a known booking time and staff member, and can be set to a daily, weekly or fortnightly (and other) recurrence pattern, each occurrence created from the same job template. ServiceM8 Help Center, checked 25 Sep 2026.
  4. ServiceM8 recommends using only letters and numbers in Forms question text because special or non standard characters can cause errors when the form is converted to PDF. ServiceM8 Help Center, checked 25 Sep 2026.
