Set it up in ServiceM8
- In ServiceM8 open Settings > Forms > Add Form and name it Pest treatment record.
- Add each question above, matching the type (Choice means Multiple Choice) and ticking Mandatory where required is true.
- Use letters and numbers only in question wording, since ServiceM8 warns special characters can break PDF generation.
- Turn on Asset Management and create an asset type for the property if you service it regularly, so 3 years of treatment history is searchable against one address.
- Set a data retention practice for at least 3 years on this form's records, since that is the minimum keeping period the Regulation sets.
What triggers the record keeping duty
Under the Pesticides Regulation 2017 (NSW), a pest management technician must make a record of every pesticide application made as part of a job or business: treating interior rooms, sub floor areas or roof cavities for pests such as termites, cockroaches, ants, silverfish, spiders or mice, or treating exterior areas including sub floors, walls, sheds, garages, fences and yards for pests such as termites, bees, wasps, ants or spiders. A narrow exemption applies for small quantities of ordinary domestic type pesticides, available at retail and applied by hand, but a pest control business working to trade quantities and professional products falls outside that exemption on almost every job.
The form below follows that fact sheet's field list directly, because the fields it asks for are the ones a NSW EPA inspection will ask to see, not a general best practice list.
What has to be in the record, and by when
The record must include the full product name applied (not a generic description), the situation the pesticide was used in, the rate of application and the quantity used, a description of the equipment, the property address and the area treated (interior, exterior, sub floor, roof cavity, with room names for interior work), the date and start and finish time, and the name, address and contact details of whoever applied it and, if different, their employer. If the pesticide was applied outdoors and through the air, weather conditions (wind speed and direction, and any other conditions the label specifies) must also be recorded at the start of the job and noted again if they change significantly during it. The technician who applied the pesticide is responsible for making the record, within 48 hours of the application, and for keeping it for 3 years.
Recording the right person as owner or occupier
The Regulation asks for the name, address and contact details of the owner or the person who manages or controls the place where the pesticide was applied, not necessarily whoever answered the door. For a tenanted property either the owner or the managing agent can be recorded. For a vacant property, the fact sheet's own guidance is to record whoever ordered or contracted the work, typically a managing agent or property manager, since that is generally who controls the premises in that situation.
What this form will not do
- This form is built to the NSW Pesticides Regulation 2017 fact sheet. Queensland, Victoria and other states set their own record keeping rules, which may ask for different or additional fields.
- The domestic like use exemption for small retail quantities applied by hand is not built into this form as a skip logic branch. A technician working under that exemption still needs to judge whether it applies.
- ServiceM8 Forms do not calculate whether an application falls inside or outside the exemption. That judgment call stays with the technician.
- The form does not separately record an APVMA product registration number. The Regulation's own fact sheet asks for the full product name, not that number, so it is not listed here as a required field.
