ServiceM8 form

ServiceM8 form for the Queensland electrical safety certificate

This ServiceM8 Form produces the Queensland certificate of test required by the Electrical Safety Regulation 2013 (ss 26 and 227). It captures the customer's name and address, what was tested, the test date and your contractor licence number, then becomes a PDF you email from the job diary as soon as practicable.

Set it up in ServiceM8

  1. Go to Settings > Forms > Add Form, name it QLD certificate of test and give it a badge name your office will recognise.
  2. Add the questions above in order. Choice means Multiple Choice and Multiple choice means Multiple Choice (Multi-answer). Avoid brackets and slashes in the live question names, because ServiceM8 warns symbols can break the PDF.
  3. Set a Skip Question If condition on Date connected to supply so it is skipped when Certificate type contains s 26.
  4. Download the sample template, lay it out as a certificate, and type your electrical contractor licence number and business name as fixed text in the header.
  5. Add both certification statements to the template and use the Certificate type field code so only the chosen one prints.
  6. Pull the customer name and job address from the job fields, falling back to the form answers when they are filled in.
  7. Create an email template called Certificate of test with the PDF attached, so sending it from the job diary is one tap.
  8. Test it on a dummy job for one s 26 and one s 227 case and check both PDFs before rollout.

Why the Queensland form ends in an email, not a lodgement

Queensland is the odd one out among the east coast states. There is no portal to lodge the certificate with and no fixed number of days to meet. Sections 26 and 227 of the Electrical Safety Regulation 2013 require the licensed electrical contractor to give the certificate to the person the work was done for as soon as practicable after testing or connection, and to keep a copy for at least 5 years.

That changes what the form needs to do. In New South Wales or Victoria the form is a data sheet that feeds a government system. In Queensland the PDF your form produces is the certificate. So the form is built to print a finished document: the four details the regulation names, a certification statement, and nothing that has to be retyped somewhere else.

The four details, and where each one comes from

The regulation asks for the name and address of the person for whom the work was performed, the equipment or installation tested, the day it was tested, and the electrical contractor licence number the testing was done under. In ServiceM8 the first comes straight off the job card. The second and third are form questions the electrician answers on site. The licence number should be typed into the Word template as fixed text rather than asked as a question, so a new apprentice cannot get it wrong.

WorkSafe Queensland dropped the signature line from its own certificate of test template in June 2023, because the regulation never asked for one. The form below still offers an optional signature, which some builders and property managers like to see, but leaving it blank does not make the certificate defective.

Proving when you gave it

"As soon as practicable" is a judgement, so your best defence is a record of when the certificate left your hands. When the electrician finishes the form, ServiceM8 saves the PDF to the job diary, and emailing it from there leaves the sent email in the same diary. That entry, with its date, is your evidence of delivery, and the PDF beside it is your 5 year copy. For the full rule set, including penalties, read the Queensland electrical safety certificate guide.

Which certificate the electrician is issuing

The form opens by asking whether this is a certificate of testing and safety (section 26, work on electrical equipment) or a certificate of testing and compliance (section 227, an installation connected to supply after work). The two share the same four details but carry different certification wording, so the answer drives which statement prints. Most switchboard, circuit and new build jobs are section 227.

What this form will not do

  • ServiceM8 does not send the certificate by itself when the form is finished; someone still has to email it from the diary.
  • The form records that tests were done. It does not hold the readings for every circuit; use a separate test record form for those.
  • Keeping the PDF in ServiceM8 only covers the 5 year retention while your subscription stays active. Export before you ever close the account.
  • This page is a form, not legal advice. The regulation text in force on the day of the work is what counts.
