This ServiceM8 Form records the AS 1851 routine service of one portable fire extinguisher: type, size, location, gauge or weight check, the service level done (six monthly, yearly or five yearly), whether the maintenance tag was punched, and defects. Set it up in ServiceM8

  1. Turn on Asset Management and create an asset type called Fire extinguisher with fields for type, size and year of manufacture.
  2. Build the form in Settings > Forms > Add Form with the questions above, then link it to the Fire extinguisher asset type so it is suggested when a unit is scanned.
  3. Add Skip Question If conditions: Weight is skipped unless Gauge reading equals No gauge (CO2), and Replacement unit serial is skipped unless Result equals Condemned and replaced.
  4. In the asset type's template setup, choose location, type, size, service level, result and date serviced as the columns for the service report and Asset Register.
  5. Create a Form Follow Up automation with the condition Result equals Condemned and replaced, created on job completion, so the office gets one list of replacements per visit.
  6. Set a recurring six monthly job per site with the Fire extinguisher badge applied.

Stamp the service level on every unit

Extinguishers on one site are rarely on the same cycle. Most units get the routine six monthly check, but a unit bought two years after the rest reaches its longer service at a different time, and a CO2 unit is checked by weight rather than by gauge. Published summaries of AS 1851:2012 describe the six monthly routine as accessibility, pressure, physical condition and seals, weighing CO2 units, and punching the date on the metal maintenance tag, with further services at longer intervals (five yearly and beyond) that are more invasive.

So the key question on this form is Service level performed, answered for each unit. When it prints in the site record the client sees at a glance which units had only the routine check and which had a pressure test or recharge, and the next technician knows which units are due for the longer service. The tag question matters for the same reason: the punched tag on the unit and the service record should always agree.

Why extinguishers are the textbook ServiceM8 asset

ServiceM8's own help centre uses a Fire Extinguisher Inspection form as its example of completing forms against assets. Each extinguisher gets a QR label, the technician scans it, and every question shows that unit's previous answers, which makes a weight that has been dropping over two services easy to spot. Units on a site show a green tick once their form is done on the job, so the technician can see which ones are still to be found.

At the end of the job the Asset Register (Settings > Assets > Asset Register) prints every asset for the customer with the answers you choose as columns: location, type, size, service level, result and date. That PDF is the site service record. For units needing replacement, a Form Follow Up automation set to run on job completion gathers every failed extinguisher from the visit into a single Inbox message, rather than one per unit.

Who needs the record

Fire extinguishers are a prescribed fire safety installation on the Queensland occupier's statement and an essential safety measure in Victoria, so the occupier or owner will be asked to produce these records. Book the next visit as a recurring job every six months so the record never has a gap.

What this form will not do

  • ServiceM8 cannot work out which service level is due from the year of manufacture. The technician chooses it; a custom report or asset field can flag units due for the longer service.
  • Service frequencies and checks in the notes come from published summaries of AS 1851:2012, which is paywalled. Check them against your copy.
  • The form does not print or punch the physical maintenance tag, which still has to be done on the unit.
  • For a customer with several sites, use the Client Sites add-on so the technician's asset list shows only the units at the site they are standing in.
