ServiceM8 form
Free ServiceM8 TMV service record form
This ServiceM8 Form records a routine thermostatic mixing valve service: the valve, the delivery limit that applies (45°C or 50°C under NCC 2025 Volume Three B2D5), a temperature reading at every outlet it serves, the cold isolation test, parts replaced and the next service date. It suits plumbers servicing aged care, hospitals and schools.

Set it up in ServiceM8
- In ServiceM8 go to Settings > Forms > Add Form, name it TMV service record and give it a badge name such as TMV.
- Add each question above in order. Choice means Multiple Choice, Multiple choice means Multiple Choice (Multi-answer). Keep question names to letters and numbers, so write 45 C, not 45°C.
- On each outlet 2 to 6 question, add a Skip Question If condition: Number of outlets tested, Less than, the outlet number.
- Click Download Sample Template to get a Word file with every Template Field Code, lay the outlets out as a table (location, temperature, limit), and add the mergefields image_customer_signature and customer_signature_date where the facility signs.
- Upload the template to the form, then turn on the form badge on the service job so the form appears on the tech's To Do list.
- If you use Asset Management, create an asset type called Thermostatic mixing valve and link this form so each valve keeps its own service history.
- Run it on a dummy job with a valve serving two outlets and one serving four, and check that the skipped outlets leave the PDF tidy.
Why the form starts with the building, not the valve
The number that matters on a TMV service is the delivery temperature at the outlet, and the limit depends on where the outlet is. NCC 2025 Volume Three, clause B2D5, caps heated water at sanitary fixtures at 45°C in the residential part of an aged care building, a patient care area in a health care building, the parts of an early childhood centre or school used by children, and designated accessible facilities, and at 50°C everywhere else. Clause B2D6 then says the 45°C areas must use a thermostatic mixing valve or a thermostatically controlled tap. A tempering valve is only an option for the 50°C cases.
So the first choice on this form is the facility type, and the second is the limit that follows from it. A tester who records 47°C at a hand basin in a nursing home has a fail, and the form should make that obvious on site, not when the facility manager reads the PDF a week later.
One reading per outlet, not one per valve
Plumbers who service one TMV that feeds six basins often write a single temperature on a job card. Facility managers in aged care and hospitals want to see each outlet, because a long dead leg or a failing mixer on one fixture shows up only at that fixture. The form below gives each outlet its own location and temperature question, and uses ServiceM8's Skip Question If condition so a valve serving two outlets does not force the tester through six.
ServiceM8 supports Equal to, Not equal to, Greater than, Less than, Contains and Does not contain as skip conditions. Set outlet 3 to be skipped when the number of outlets is less than 3, outlet 4 when it is less than 4, and so on.
What the cold isolation test and the storage side tell you
The field testing and maintenance procedure for TMVs is in AS 4032.3, whose full title covers field testing, maintenance or replacement of thermostatic mixing valves, tempering valves and end of line temperature control devices. The standard is sold by Standards Australia, so the form does not repeat its step by step method. It records the result of the fail safe test your procedure calls for (isolating the cold supply and confirming the hot flow shuts off or drops to a safe level), plus the temperature seen during that test.
The form also records the supply temperatures, because a TMV is only half the story. The ABCB warns that Legionella grows in water between 20°C and 45°C and that growth is reduced significantly in water stored at 60°C and above. A hot supply reading well under 60°C at the valve inlet is worth a note to the facility even when every outlet passes.
How it fits ServiceM8
Build it with the Forms add-on and link it to the valve as an asset if you use Asset Management, so each TMV keeps its own service history. Completed forms become a PDF in the job diary. If the facility wants its own staff member to sign after reading the record, put ServiceM8's customer signature field in the Word template instead of a Signature question: the app produces the PDF, the client reviews it, taps Sign Document, and the signature lands on the PDF. ServiceM8 notes this review and sign flow only works when the form is completed in the mobile app.
What this form will not do
- ServiceM8 Forms do not check a reading against a limit, so the tester answers Any outlet over the maximum by hand.
- The form does not reproduce the AS 4032.3 test method, which is sold by Standards Australia. Keep your procedure and the valve maker's instructions to hand.
- Service intervals are not set here. Use the facility's water management plan, the standard and your state plumbing regulator's rules.
- Nothing is lodged with a council or regulator by this form. Check whether your state requires a separate notice for the work.
- Facility review and signature on the PDF works only when the form is completed in the ServiceM8 mobile app.

- NCC 2025 Volume Three B2D6 requires a temperature control device delivering water under B2D5(a) (the 45°C cases) to be a thermostatic mixing valve or a thermostatically controlled tap; for B2D5(b) a tempering valve or temperature limited water heater is also allowed.
