Set it up in ServiceM8

  1. In Settings > Forms > Add Form, name the form Form 4 capture and set Badge Requirement to mandatory so the job cannot be completed without it.
  2. Add the questions above in order. The categories question is Multiple Choice (Multi-answer) with the twelve categories written exactly as listed.
  3. On every question after Is this notifiable work, add Skip Question If with ANY conditions: Is this notifiable work equals Permit work, or equals Minor work.
  4. On the water heater and backflow questions, add Skip Question If: Categories of notifiable work does not contain 6 Water heaters (or 7 Backflow prevention devices).
  5. Download the sample template and order the fields to match the Form 4 so the office can copy them straight into the QBCC Portal.
  6. Add the Form 4 capture badge to your job templates for hot water, bathroom and fixture jobs.
  7. In the QBCC Portal, set up your office administrator as a nominated representative so they can lodge from the completed PDF.

What notifiable work is, and why the form exists

In Queensland, most plumbing and drainage in existing homes, and some in commercial buildings, is notifiable work: you do it without a council permit or mandatory inspection, then register it with the QBCC afterwards. Replacing a hot water system, relocating a toilet or renovating a bathroom are the everyday examples. Work for a new building is permit work, not notifiable work.

Registration is on the Form 4 (Form 4a for public sector clients) and it has a hard deadline: within 10 business days of issuing an invoice for the work, and again for each progress invoice on a staged job. Lodging online through the QBCC Portal costs $36.88; the paper form costs $49.80. Not registering, and not giving the customer their copy, are each offences with a maximum penalty of 60 penalty units. The practical risk for a busy plumber is not the form, it is the office not knowing the job needed one.

The 12 categories as a picklist

The Form 4 asks you to tick every category that applies, and the QBCC says you must select all of them. Schedule 1 of the Plumbing and Drainage Regulation 2019 sets the categories: supply pipes, fire services, sanitary plumbing, sanitary drainage, temperature control devices, water heaters, backflow prevention devices, greywater treatment plants, fixtures in Class 1 or 10 buildings, fixtures in Class 2 to 9 buildings of one or two storeys, sealing sanitary drains and sealing supply pipes. A single hot water replacement often ticks three: supply pipework, the water heater and its temperature control device.

In ServiceM8 that is a Multiple Choice (Multi-answer) question, so the technician ticks every category on site while the work is fresh. Put a first question asking whether the job is notifiable, permit or minor work, and skip the rest of the form for the last two.

Hand the office a finished lodgement

The licensee can nominate a representative, such as an office administrator, to register notifiable work on the QBCC Portal on their behalf. This form is built for that split. The technician answers the questions and takes the photos; the office opens the completed PDF in the job diary and copies each answer into the portal. Attach the form to the job with a mandatory badge so a job cannot be invoiced without it, and after lodging, email the customer the QBCC copy within 20 business days, as the QBCC requires.

What this form will not do

  • ServiceM8 does not lodge with the QBCC. The Form 4 is still registered and paid for on the QBCC Portal, or by the paper form.
  • The 10 business day clock starts at invoice, which ServiceM8 does not link to this form. A follow up badge or automation is needed if you want a reminder.
  • Whether a job is notifiable, permit or minor work is the licensee's call. The QBCC tool and guide are the authority, not this form.
  • Fees quoted are the QBCC's and change each 1 July. The form carries no fees.
