Set it up in ServiceM8
- Download the current approved gas compliance certificate from the Queensland regulator's website and note its exact field names.
- Build the questions above in Settings > Forms > Add Form, renaming any to match the approved form's wording.
- Download the sample template and lay it out to mirror the approved certificate, then add the Customer Signature merge field where the owner signs.
- Add a Skip Question If on the test and evidence questions so a No, safety concern answer ends the form and prompts the technician to issue the safety notice instead.
- Create an email template called Gas certificate to network operator, and a job queue or follow up badge so the office sends it within 5 business days.
- Test on a job in the mobile app: complete the form, have a colleague sign on screen as the customer, and confirm the signed PDF replaces the draft in the diary.
Gas certificates come from gas law, not plumbing law
Many Queensland businesses hold both plumbing and gasfitting licences and run both kinds of job from ServiceM8, so it is tempting to treat the gas certificate as a variation of the plumbing paperwork. It is not. Plumbing notifiable work is registered with the QBCC under the Plumbing and Drainage Act 2018. A gas compliance certificate is required by section 734(3) of the Petroleum and Gas (Production and Safety) Act 2004, and the details of who gets it and when sit in the Petroleum and Gas (Safety) Regulation 2018. Different Act, different regulator, different approved form, and a maximum penalty of 300 penalty units under the Act for failing to give it.
When a licensed gasfitter installs all or part of a gas system away from an operating plant (a house, a shop, a restaurant kitchen), section 95 of the regulation says the certificate goes to the owner, operator or proposed operator when the installation is finished, and to the relevant network operator for the gas system within 5 business days. So one job produces two deliveries on two timelines, and the form has to support both.
Build it around the approved form
The certificate itself must be in the approved form, so treat this ServiceM8 Form as the capture tool and the approved form as the output layout. Download the current approved gas compliance certificate from the Queensland regulator's website, lay your Word template out to match it field for field, and map each ServiceM8 answer into its matching box. The questions below cover what an installation certificate usually needs; add or rename any to match the approved form's exact wording.
Because the owner receives the certificate on completion, use ServiceM8's review before signing option rather than a Signature question. Set the template up with the Customer Signature field, and when the form is finished in the app ServiceM8 shows the customer the finished PDF, they tap Sign Document, and the signed certificate lands in the job diary ready to email to the owner and the network operator.
When the job finds a problem instead
Section 734A of the Act applies when an installer knows or suspects a gas system does not comply or poses an imminent risk. The installer must give the owner or operator a notice in the approved form, take measures to remove the risk (isolating a leaking appliance is the Act's example), and phone an inspector and the network operator immediately for an imminent risk. That is a different document, so this form ends with a question that routes the job to it rather than issuing a certificate.
What this form will not do
- The approved form's exact layout and wording could not be reproduced here. Match your template to the current version from the Queensland regulator before use.
- Customer signature after review only works in the ServiceM8 mobile app, not in the online dashboard.
- ServiceM8 does not send the network operator copy on its own. The 5 business day step needs an email template and a reminder.
- This page covers installation certificates. Vehicle and vessel gas systems, operating plants and interstate certificates follow other sections of the regulation.
