Medicare chronic condition management allied health items 2026-27

Thirteen Medicare items, one per allied health profession from 10950 to 10970, share the same $74.55 schedule fee ($63.40 rebate at 85 per cent) and the same rules: a GP chronic condition management plan, at least 20 minutes, up to 5 services combined across all of them in a calendar year. Podiatry is 10962, chiropractic 10964, osteopathy 10966, physiotherapy 10960, psychology 10968.

What changed this year

  • All 13 individual chronic condition management items (10950 to 10970) carry the identical $74.55 schedule fee, effective 1 July 2026, whatever the profession. Podiatry, chiropractic, osteopathy, physiotherapy, dietetics, exercise physiology, occupational therapy, psychology, speech pathology, audiology, diabetes education, Aboriginal and Torres Strait Islander health work and mental health work are all priced the same.
  • On 1 July 2025 the GP Management Plan and Team Care Arrangements that used to unlock these items were replaced by a single GP chronic condition management plan. Patients with a pre-1 July 2025 GP Management Plan and Team Care Arrangement can keep using it until 30 June 2027, after which a chronic condition management plan is required.
  • The 5 service cap is shared, not per discipline. A patient can take all 5 in one profession or spread them across several, for example 1 dietetic and 4 podiatry services in the same calendar year, and Aboriginal and Torres Strait Islander patients can access additional services under the separate MBS Group M11.
  • Case conference items (10955, 10957, 10959) are not profession specific. Any eligible allied health professional or Aboriginal and Torres Strait Islander primary health care professional participating in a community or residential aged care case conference bills the same three items, banded by duration.

Same fee, same rules, 13 professions

Medicare's chronic condition management items are the closest thing the MBS has to a single allied health fee schedule. Podiatry (10962), chiropractic (10964), osteopathy (10966), physiotherapy (10960), dietetics (10954), exercise physiology (10953), occupational therapy (10958), psychology (10968), speech pathology (10970), audiology (10952), diabetes education (10951), Aboriginal and Torres Strait Islander health work (10950) and mental health work (10956) all bill $74.55, all pay an 85 per cent rebate of $63.40, and all need the same GP chronic condition management plan, referral and 20 minute minimum. Writing a separate page for each of these would repeat the same fee and the same rule five times over; the difference between them is only the item number and the profession named in the item's own description.

This is different from the Better Access mental health items (which scale by session length and registration type) and from the diabetes group services (which have their own item series and session caps). Those get their own pages in this batch.

One shared cap of 5 services a year, however you split it

The 5 service cap does not reset per profession. A patient with a chronic condition management plan gets up to 5 MBS-supported individual health services across all 13 items combined in a calendar year, whether that is 5 sessions with one podiatrist or a mix of 1 dietetic visit and 4 physiotherapy visits. This is the detail multidisciplinary practices most often get wrong: booking a new patient in for 3 physiotherapy sessions without checking whether they already used 2 with a psychologist elsewhere leaves only what remains of the shared 5, not a fresh 5 for physiotherapy.

Practices can check remaining services through the MBS Items Online Checker or a patient's Medicare Online Account before committing to a course of treatment that assumes the full cap is available.

The 2025 plan rename, and what it means for referrals now

From 1 July 2025, the GP Management Plan and Team Care Arrangements pairing that used to unlock these items was replaced by a single GP chronic condition management plan. It is a genuine simplification, not just a rename: one plan replaces two documents. Patients who already had a GP Management Plan and Team Care Arrangement before that date can keep using it until 30 June 2027, so a clinic will see both plan types referenced in referrals for some time yet.

Referrals written from 1 July 2025 onward also changed: they are valid for whatever period is stated, or 18 months from the first service if no period is given, rather than needing to state a fixed number of services. Older referrals still must state a service count and stay valid until they are used up.

Case conferencing sits outside the profession-specific items

Items 10955, 10957 and 10959 are not tied to any one profession. Any eligible allied health practitioner, or eligible Aboriginal and Torres Strait Islander primary health care professional, attending a community or residential aged care multidisciplinary case conference bills the matching duration band, from $58.50 for 15 to 20 minutes up to $166.85 for 40 minutes or more. A practice that only sets up its Medicare items by discipline will miss these, since they apply equally whether the attending clinician is a physiotherapist, psychologist or podiatrist.

Claiming notes

Load these fees into your practice software

  1. Because every discipline shares the same $74.55 fee, most single-discipline clinics only need one Medicare billable item, but a multidisciplinary practice should still create one named item per profession (for example "Medicare 10962 Podiatry CDM" and "Medicare 10964 Chiropractic CDM") so claims and reports track correctly against the right item number even though the price is identical.
  2. Under Settings, then Data & docs, then Data imports, use Billable items to bulk load the CSV rather than typing each profession's item code by hand across a multidisciplinary roster.
  3. CSV headings: Item code, Name, Price (ex. tax), Taxable, Item type. Put the 5-digit MBS number in Item code, the fee in Price (ex. tax), and service in Item type. Individual CDM services are GST-free under the health exemption, so leave Taxable blank.
  4. Add the three case conference items (10955, 10957, 10959) once, shared across all practitioners, rather than duplicating them per discipline, since they are not profession specific.
  5. Every item on this page bulk bills through Cliniko's Tyro Health (formerly Medipass) connection, the standard Medicare claiming route in Cliniko regardless of which of the 13 professions is billing.
  6. Set a recall or task in Cliniko to flag when a patient nears their 5 service cap for the calendar year, since the cap is shared across every discipline the patient sees, not reset per practitioner.
